    Become a plague carrier when you use the Surgeon’s Visage and gain the Contagion Curse. The Curse weakens you but you can spread the Plague for 5 minutes and remain immune to the Plague for 10 minutes.

    Basically, when you use this item it gives you the Contagion Curse, and by getting close to another player you can give it to them! :)
